How to haggle over medical bills
You have more options than you might realize with health care bills. If you know how to cut down the original billed amount, you can save a great deal.
Big medical bills can be frightening if you don't have insurance or don't have enough coverage. And frightened people can do stupid things, like hang up on someone who could help them.
Nicholas Newsad discovered this when he was an administrator for a surgery center in Ohio.
"I was amazed at how many patients wouldn't talk to us. . . . Quite a few just hung up when I called," said Newsad, who's now a senior associate at HealthCare Appraisers, a Castle Rock, Colo., company that values medical practices and services. "We had financial assistance and policies that would help them, but we couldn't help them because they wouldn't talk to us at all."
Newsad's experiences prompted him to write a book, "The Medical Bill Survival Guide: Easy, Effective Strategies for People Experiencing Financial Hardship," that outlines strategies for coping with medical debt.
A key point is that health care bills are different from most other debt:
- Many people qualify for financial assistance that can dramatically reduce or eliminate their bills.
- Most people can negotiate big discounts off the sticker prices of their treatments.
- Health care providers often offer no-interest payment plans.
- Even after bills have been turned over to collectors, people often have more options than they think.
Of course, you may be facing a mound of bills so high you could never pay them all off. But most people can get a better deal if they try. Here's what you need to know:
Many bills contain errors. Hospital bills, particularly, can include charges for procedures that were never done, doctor visits that never happened and drugs that weren't administered. Ask for an itemized bill, and go through it line by line. You can use the Healthcare Blue Book, a free consumer guide, to see if charges might be excessive. If you find errors, alert the billing department.
One of my Facebook fans said the hospital bill for the birth of her daughter contained a number of billing mistakes.
"There were charges for pain/sleeping pills I never took . . . and some other dubious stuff," she wrote. "I went to billing office . . . and we went over line-by-line. Got several things knocked off."
Be alert if you don't get a bill after a visit to a medical provider. The provider may be sending the bill to the wrong address or billing the wrong insurer. The bill could be turned over to collections without your knowing, and collections can hurt your credit scores. Don't assume that no news is good news. If you haven't been billed or you keep getting billed for something your insurance should have paid, follow up to find out what's going on, and try to fix it before the bill ruins your credit.
If your bills are significant, you may want to hire help to review the charges and negotiate discounts. Medical Cost Advocate, Health Advocate and Chapman Consulting are three companies that offer these services.
Most hospitals and many other providers offer charity care. If your income is low, ask to speak to a financial counselor about charity assistance. Hospitals typically base the amount of help you're offered on the federal poverty line, which is currently just under $11,000 for a single person and $22,350 for a family of four.
Those whose incomes are less than twice the poverty-line limits often have their entire bills erased, Newsad said. That means you could make up to about $22,000 as a single person, or nearly $45,000 as a family of four, and be excused from the debt.
Even those who make more can get big discounts.
"If you're at 300% (of the poverty line), you get 75% off," Newsad said. "At 400%, you get 50% off."
Your medical providers may not volunteer that such help is available. You often have to ask. Even if your provider doesn't offer financial assistance or you make too much to qualify, discounts are still available. Read on.

The paying cash upfront is a great way to save money. I chose to have a sub lingual frenectomy on an ongoing suggestion by my dentist. None of my insurance covered it. I was in a position to pay for it and I half jokingly asked if there was a cash discount. There was and it had to be paid upfront before the surgery. It saved me a little over $100.00 or about 20%. So now I ask for a cash discount for nearly everything I buy. Garage doors, gutters, home repairs, medical, etc. It's been as little as 5% to as high as 30%.
I work in the medical field and for hospitals and clinics there are a lot of people that give false information and know how to hide from bill collectors. They have or give false SSNs, employers, addresses, phone numbers, bank accounts, references, etc. The percentage of nonpayers is from 25% to as high as 60%. That depends on where and in what city they are located. Some are worse than others for obvious reasons. So those that can pay or have insurance are charged more to cover that loss from those that feel entitled to cheat. They have to get the money from somewhere to stay in business. There is no way to provide a medical service if you are spending more than you take in. Not like the government which seems to have found a way to do just that.
Although the writer probably has the best intentions, he failed in this article. Healthcare costs skyrocket because there are too many people that are cheating the system. Let's say you sprain your ankle and you choose the emergency room. You incur the following costs. There has to be a receptionist staffing the ER, you have a triage nurse to take all your vitals, you get brought back to a "bed" that has to be flexible to all sorts of patients and is very expensive for the hospital to purchase, the cost of cleaning the linens from the beds you sit on, you are then taken care of by the nursing staff and seen by a qualified provider (MD, NP, or PA) that is trained in emergent care (more training for the MD, more schooling costs, thus the more the hospital has to pay that person). Then you have to pay for the likely round of tests. Because people sue over the slightest, the medical personnel have to cover themselves. So, what looks like a sprain has to be treated as a potential break. So, that means xrays. Xray machines cost thousands of dollars. The radiologist who reads the test has to be paid because you don't want someone not trained reading your test. Then you have to have someone on staff at all times in the pharmacy to cover so that you can get some pain relief. Then there is the cost of business, buildings, lights, etc. All of this costs the hospital money. So, let's say the person before you doesn't pay. Who pays? You do. If you don't pay, then it has to be passed on. If the hospital does not bring money in, it can not stay open. That is the law of any business. Oh, and it isn't coming from the government who recently passed a bed tax to tax hospitals to pay for medical care (how screwed up is that?!) So, yes, the prices get jacked up to cover for those they know can't pay. So, there are those who may be less fortunate. There are some of these options in place to help them. But there are many more who can afford a portion don't pay because they feel entitled to free healthcare. It is mostly these people who are looking for articles like this. Those who can't afford to pay are the ones I see trying the most to pay what they can. In summary, pay your portion as long as you are able to. Help keep the system going so that when you need healthcare it is there. Don't try to cheat the system, because it will only continue towards the rising of healthcare costs!!!
I recently had a spinal epidural for a back problem. TWO different billing companies are now billing me for the EXACT same thing!
Which one do I pay? What happens if I pay one of them and the other one keeps billing me? Problem is that I see patients myself and have really no time (except half hour lunch when the billing companies are not answering the phone) to sit on the phone and go through two minutes of "press one, etc." to get to a real live person to explain the double billing. I cannot carry my cell phone around to answer personal phone calls. Then when/if you DO get a real live person on the phone, they are not a medical professional who understands the medical language on the statement. The physical therapy that I have been getting is now generating 5 different bills for each session. I am not even sure that I am really getting some of the procedures that I am getting billed for. It is ridiculous!
The tips provided in this article may certainly work for some patients, and while many of us simply don’t like to “haggle” over the cost of anything, it’s becoming increasingly clear to many patients that they need to negotiate with providers in order to avoid severe economic hardship after they’ve received even minor medical care. A great number of patients figure out on their own that health insurance companies pay a much lower rate based on contractual agreements, and as this article mentions, this really is a good starting point for addressing the “fairness” of medical bill costs with a provider. Good negotiation goes much further than that. When patients pick up the phone and talk frankly to providers about expectations, payment plans, and the like, the two parties can often come to an agreement that can help the patient to avoid the last resort of bankruptcy.
